Enrollment & Forms

PYDC enrolls children 6 weeks through the age of 8 years old without regard to race, color, culture, gender, religion, national origin, ancestry, sexual orientation, political affiliation, income, or disability. A child is eligible to enroll if an opening exists in the age-appropriate room. If no opening exists in an age-appropriate room, the child is placed on a waiting list, pay a non-refundable $50 hold fee, and admitted as soon as space becomes available. Prior to enrollment, parents will meet with the director to tour the facility and will receive an enrollment packet including the forms required by the Iowa Department of Human Services. At least one week prior to the child’s first day, parents must complete, sign, and submit the following:

·         Enrollment application

·         Emergency medical consent

·         Child physical exam report

·         Immunization certificate record

·         Travel/activity/photo release

·         Pick-up permission

·         CACFP enrollment form

·         Contract for care

·         Receipt of handbook

Enrollment information and forms submitted are confidential and accessible only to administrative staff and licensing officials. Annual updates to all child files are required. If a child’s file is not updated or is missing information, PYDC reserves the right to terminate services for the child.
